Jill Poser, CGCM, CMC, CDCP
Jill Poser is a national leader in Care Management, Life Care Planning and Private Duty Home Care creating heart felt experiences for people to age-in-place.
With over 15-years of professional experience in the senior space and her personal journey caring for her own parents led Jill to successfully establish a full-service care management and private duty home care concierge company.
Jill and her team create a safe space for their clients and families to experience a unique approach toward their care and well-being through the principles of Life Care Planning that coordinates the use of money with their changing care needs to achieve sustainable outcomes.
She strongly believes in the power of the Mind - Body connection in creating positive long-term health care solutions and is dedicated to the ongoing development of a strong team of multidisciplinary medical and care management professionals, assuring the highest quality of service is provided to all clients.
Jill is Care Manager Certified (CMC) through the National Academy of Certified Care Managers (NACCM) and a Certified Geriatric Care Manager (CGCM) through the International Commission of Health Care Certification (ICHCC), an internationally recognized certification in care management. She actively maintains her credentials while adhering to all continuing education requirements, the Code of Ethics and Standards of Practice. The ethical principles at the core of the Code of Ethics are the foundation for the Standards of Practice and set a national standard for the very practice of care management.
Through her ongoing commitment to client advocacy, Jill has most recently become a Certified Dementia Care Partner (CDCP).

Denise Silvestri
With over a decade of experience in a care management delivery service model, Denise Silvestri serves as the Administrator of Private Duty Home Care Concierge. In her role as Administrator, she oversees daily operations, compliance and regulations, policy and procedure development and implementation, general support for the clinical team and the principal; all with the goal toward improving the client experience.

Jessica Patino, RN, NCM, CDCP
Jessica Patino is a Registered Nurse with over eighteen years of professional experience in the older adult space. Her knowledge and hands-on qualifications span multiple healthcare settings that include care management, critical care, hospice, community nursing, home health, and private duty home care services.
In her role as Director-Clinical Services, Jessica oversees and supports the Life Care Management team implementing processes and programs that continue to elevate the PDHCC client experience.
Through her continuing commitment to client advocacy, Jessica has most recently become a Certified Dementia Care Partner (CDCP).
Senior Life Care Manager
Liz Casper, MG, CPT, CMC, CDCP, CDCS
Liz Casper is a Senior Life Care Manager with over fourteen years of professional experience in the older adult space. Her expertise and practical training stretch over many disciplines that include life care management, therapeutic recreation, personal training, gerontology, and private duty home care services.
In her role as Senior Care Manager, Liz works closely with clients and the PDHCC Medical Care Management team in transition care management, life care planning, therapeutic recreation, active aging, and dementia care coordination. Within the PDHCC practice, Liz spearheads pet therapy and implements individualized active aging and fall prevention protocols that foster independence.
Liz is Care Manager Certified (CMC) through the National Academy of Certified Care Managers (NACCM), a Certified Personal Trainer through the American College of Sports Medicine (ACSM), a Certified Dementia Care Partner (CDCP).
Because of her continual commitment to active aging, therapeutic recreation and dementia care practices, Liz has become a Certified Dementia Engagement Specialist.
