Jill Poser is a national leader in Care Management, Life Care Planning and Private Duty Home Care creating heart felt experiences for people to age-in-place.

 

With over 15-years of professional experience in the senior space and her personal journey caring for her own parents led Jill to successfully establish a full-service care management and private duty home care concierge company. 

 

Jill and her team create a safe space for their clients and families to experience a unique approach toward their care and well-being through the principles of Life Care Planning that coordinates the use of money with their changing care needs to achieve sustainable outcomes.

 

She strongly believes in the power of the Mind - Body connection in creating positive long-term health care solutions and is dedicated to the ongoing development of a strong team of multidisciplinary medical and care management professionals, assuring the highest quality of service is provided to all clients.

 

Jill is Care Manager Certified (CMC) through the National Academy of Certified Care Managers (NACCM) and a Certified Geriatric Care Manager (CGCM) through the International Commission of Health Care Certification (ICHCC), an internationally recognized certification in care management. She actively maintains her credentials while adhering to all continuing education requirements, the Code of Ethics and Standards of Practice. The ethical principles at the core of the Code of Ethics are the foundation for the Standards of Practice and set a national standard for the very practice of care management.

 

Through her ongoing commitment to client advocacy, Jill has most recently become a Certified Dementia Care Partner (CDCP).
